Assessment
This assessment is designed to help us understand some of the issues you and your child are dealing with. Our goal is to use this information as a starting point to determine if and how we can help your child. Please answer the following questions as honestly and accurately as you can. Towards the bottom is an area where you can expand on what your family is currently experiencing. Your results will be reviewed and will give us an indication of what is recommended for your child.
